Partnering to deliver social health services
The Oregon Health Plan’s (OHP) Health Related Social Needs (HRSN) benefit was developed to support OHP/Medicaid members who are experiencing certain life transitions and have specific medical needs. You learn more about HRSN benefits here: Health Share of Oregon | Health Related Social Needs.
Community Capacity-Building Funding (CCBF) is an opportunity by the Oregon Health Authority to support Coordinated Care Organizations (CCOs) to invest in community-based organizations (CBOs) who are interested in providing HRSN benefits. These grants will allow qualifying CBOs to build the workforce and infrastructure needed to become a HRSN provider.

As a CCO, Health Share will be responsible for reviewing CCBF applications, awarding funding, and monitoring the use of funding.
2025 CCBF Timeline
March 3 – May 30: Technical assistance is available to all CCBF applicants
April 1 – May 30: Application window
No applications submitted after May 30, 2025 at 12:00 PM will be considered.
September 30: Anticipated award notification date
How to Apply
Organizations interested in applying to 2025 CCBF can do so by creating an account with Health Share’s electronic grant portal, WizeHive. The WizeHive link will be posted on this webpage on April 1, 2025.

Applications must be submitted through the WizeHive online portal by May 30, 2025 at 12:00 PM PST. Applications will be evaluated based on how they fit into the 2025 CCBF Funding Priorities and the following Application Evaluation Domains:
- Completion: application materials are filled out and the applicant meets minimum eligibility requirements
- Need: application aligns with Health Share’s funding priorities, responses include a strong justification for why CCBF is needed to support delivery or bolster capacity to support HRSN (including clear articulation for additional funding if applicant received funds in 2024) and requests permissible uses of funding
- Experience: application describes relevant experience in providing HRSN or similar services and intends to develop capacity to offer one or more HRSN services aligned with Health Share’s funding priorities
- Intent to contract: application indicates entity is currently contracting for or intends to contract to provide HRSN services. (Note: Receiving CCBF is not the same as an HRSN Service Provider contract and does not guarantee that an entity will receive an HRSN Provider contract. CCBF is intended to build necessary infrastructure for HRSN contracting.)
- Equity plan: includes details on how the application intends to promote health equity through the use of CCBF and entity serves populations eligible for HRSN benefits
- Non-duplication: application clearly articulates CCBF will not duplicate or supplant other funding sources
