HRSN Services Overview

Health Related Social Needs (HRSNs) are social and economic needs that impact an individual's ability to maintain health and well-being. Through Oregon's 1115 Medicaid waiver, the state will be able to cover new HRSN services for eligible Oregon Health Plan (OHP) members. 

HRSN services include: 

  • Climate-related resources launched March 2024
  • Outreach and engagement launched March 2024 
  • Housing supports (launching November 2024)
  • Nutritional services (launching January 2025)

HRSN Providers

Health-Related Social Needs (HRSN) providers are organizations that will provide HRSN services in a way that is culturally and linguistically appropriate, responsive and trauma-informed. All contracted HRSN providers must meet certain provider qualifications as detailed below.

HRSN Provider Qualifications

HRSN Provider Experience and Expertise: Per OHA, all HRSN services providers are expected to meet certain qualifications that ensure they are capable of providing high-quality services to qualifying members as well as have culturally specific expertise to connect with members of priority populations. Qualifications may include, for example: 

  • Be accessible to HRSN participants, including having the operating hours and the staff necessary to meet participant need.  
  • Demonstrate their ability and/or experience with effectively serving at least one of OHA’s Priority Populations.  
  • Provide culturally and linguistically appropriate, responsive, and trauma-informed service delivery, including by ensuring their ability to:  
  • Demonstrate a history of responsible financial administration via recent annual financial reports, an externally conducted audit, and/or other similar documentation. 
  • HRSN Service Providers must have the ability to fulfill all obligations related to participating in the Closed Loop Referrals process (acceptance and confirmation). 

Additional Requirements for Housing Services Providers:  

  • Housing services providers must have knowledge of principles, methods, and procedures of housing services covered under the waiver, or comparable services meant to support individuals in obtaining and maintaining stable housing. 

Additional Requirements for Nutrition Services Providers:  

  • Nutrition services providers must have knowledge of principles, methods, and procedures of the nutrition services covered under the waiver, or comparable services meant to support an individual in obtaining food security and meeting their nutritional needs. Nutrition service providers must follow best practice guidelines and industry standards for food safety. 

Additional Requirements for Outreach and Engagement:  

  • HRSN outreach and engagement and benefit linkages providers must have knowledge of principles, methods, and procedures of these services or comparable services meant to outreach to and engage the populations covered under the waiver and connect them to benefits and services to meet their needs. 

Member Eligibility Criteria: Clinical and Social Risk Factors

At least one Clinical Risk Factor: 

  • Complex behavioral health need 
  • Developmental disability need 
  • Complex physical health need 
  • Need for assistance for ADLs / IALDS or eligible for LTSS 
  • Interpersonal violence experience 
  • Adults 65 years of age or older 
  • Repeated emergency department use and crisis encounters 
  • Pregnant/postpartum 
  • Young adults with special healthcare needs 

At least one Social Risk Factor: 

  • Housing-related needs 
  • Nutrition-related needs 
  • HRSN device needs 

Closed Loop Referrals

HRSN Service Providers must have the ability to fulfill all obligations related to participating in the Closed Loop Referrals process (acceptance and confirmation). 

Health Share has contracted with Unite Us to provide a platform known as Connect Oregon (Connect Oregon (uniteus.com)) to support a closed loop referral function. The platform supports sending HRSN requests to Health Share plan partners from providers or from members directly via a web-based platform.

Providers should submit invoices via the Connect Oregon platform.
