Your health, your choice
As a Health Share member, you can choose the health plans and providers that are right for you and your family. No matter which health plans you choose, your benefits are the same, and they’re available at no cost to you.
Your health plans and primary care provider (PCP)—that’s your main doctor—are listed on your Member ID Card. You can switch health plans if you want—just call us at 503-416-8090 or email us at info@healthshareoregon.org.
To change your doctor, dentist, or other care provider, contact your health plan.
Our comprehensive online provider directory is available now. The directory includes providers from our medical health plans, dental plans, and behavioral health plan.
Our Family of Plans
Our network gives you access to more great care through Health Share. You can choose between CareOregon, Kaiser Permanente, Legacy Health PacificSource, OHSU Health, and Providence Health Assurance.
Medical Health Plans
You can choose the plan that works best for you and your family, and that includes the providers you know and trust.
If you want to keep your plan but change your doctor or other care providers, call the contact number for your medical health plan listed on your Member ID Card.
Dental Plans
Each Health Share dental plan allows you to visit a different network of dentists. You can choose the plan that includes the dentist you want to see.

Mental Health + Substance Use Plans
Health Share's mental health and substance use provider network is managed by CareOregon. Clinics and providers are the same, and there are many options for support.
To get care and services:
- Find a Mental Health or Substance Use Provider online
- Call CareOregon at 503-416-4100
- Talk to your primary care provider (your main doctor) listed on your ID card
